Term Life
During the open enrollment period, elect or increase Guaranteed Issue coverage amounts for employee, spouse and child on the self service site. Complete the Optional Application for elections beyond the guaranteed issue limits, or when enrolling outside of the open enrollment period. Proof of good health (evidence of insurability) will be required.
Short Term Disability
During the open enrollment period, or anytime during the year, enroll by completing the Optional Application. New coverage or increases in coverage with require proof of good health (evidence of insurability).
Optional AD&D
During the open enrollment period elect or increase employee and spouse coverage Guaranteed Issue on the self service site. When enrolling outside of the open enrollment period, elections can be made by completing the Optional Application. Proof of good health (evidence of insurability) is never required for Optional AD&D.
Long Term Disability
During the open enrollment period, elect or increase coverage Guaranteed Issue. Proof of good health (evidence of insurability) is never required, however, a 2 year pre-existing conditions clause will apply to increased benefit amounts. LTD elections or increases can only be made during the open enrollment period.
HOW TO ENROLL:
Complete the Optional Application and submit it to Minnesota Management & Budget. If applicable, you will receive the Evidence of Insurability form in the mail, from the insurance company, with instructions.
NOTE: New employees and newly eligible employees have a one-time opportunity to enroll for limited amounts of coverage Guaranteed Issue, no evidence of insurability needed. New hires must enroll withing 35 days from their employment date and employees who become eligible at a later date must enroll within 30 days of the qualifying employment status change date.
