How do I file a disability claim for my employee and where should it be sent?
If your group disability insurance is with Assurant Employee Benefits, you can obtain the forms you need at www.assurantemployeebenefits.com On the home page, on the left hand side, is an Employer link for forms. Both the long term disability and the short term disability forms can be obtained from this website. Of course, Ochs, Inc. would be happy to mail you hard copies of the forms you need as well as provide assistance if you insure with any other carrier. Just contact us here.
The disability claim process begins with you completing the appropriate portion of the claim form and then printing it off and getting it to the employee (claimant). It is the employee's responsibility to get the form to the attending physician for the Physician Statement. The doctor's office should mail the form directly to the Minneapolis address listed on the bottom of page 1 of the claim form.
